What is cortisol?
Cortisol is a hormone produced by the adrenal glands under the control of the hypothalamic–pituitary–adrenal (HPA) axis. Often called the 'stress hormone', it does much more: it helps keep energy available during fasting, activity or stress, takes part in metabolism and influences alertness and the sleep–wake cycle. Its release follows a daily rhythm, highest in the morning.
Does menopause change cortisol?
Current evidence does not show that menopause raises cortisol in every woman. However, hormonal fluctuations – especially falling oestrogen – can change how we respond to stress, and many women feel that stress affects them more at this stage.
For an overview of this stage of life, see our complete guide to perimenopause.
The stress–hot flush–sleep cycle
Research such as the SWAN study has found that perceived stress is associated with how intense hot flushes and night sweats feel, although it does not cause them. Hot flushes can disturb sleep, poor sleep makes stress harder to manage, and so on. Night-time waking is one of the most common sleep changes during the menopausal transition.
Techniques that help
- Breathing: a few minutes of slow, diaphragmatic breathing can help you through a difficult moment – during the day or at night.
- Movement: combine strength training with aerobic exercise and walks outdoors. A meta-analysis of 26 trials in postmenopausal women found exercise was associated with improvements in mood.
- Sleep: keep regular times, a cool bedroom and a screen-free wind-down routine.
- Connection: talk to friends, family or a professional about how you feel.
Nutrients and psychological function
Vitamin B6 contributes to the regulation of hormonal activity and to normal psychological function. Magnesium also contributes to normal psychological function.
When to speak to your GP
If symptoms are persistent, getting worse or affecting your daily life, speak to your GP or practice nurse. They can look at all possible causes and explain the options available, including hormone replacement therapy (HRT) where appropriate.
